When an qualify acupuncturist diagnose the pateint in both way Western And TCM  see the imbalance in five elements then only he or she will insert needle to there pateint with intention to balance the chi of yin and yang or in simple language I can say balancing the energy of the pateint or five elements.beacause your body is made up of five elements and chi ... Nature knows that how to treat the body but Acupuncture give the road or right pathway to treat disease or any disorders
But question arise in your mind that how it will happen now just understand it  cosmic energy  is every were with help of needle the energy enter and  ....needle work like qi(chi,pran)
